UKRAINIAN FCU 2012 SCHOLARSHIP AND INCENTIVE AWARD PROGRAM
Eligibility and Criteria
Ukrainian FCU is pleased to make available fifteen (15) Scholarship and Merit Awards for
UFCU student members in 2012. Ukrainian FCU members in good standing, who are either
high school seniors or full-time, undergraduate college students accepted to or attending an
accredited college, university or degree-granting vocational education institutions in 2012 are
encouraged to apply for the following Scholarships and Merit awards:
$2,000 Walter Hawrylak Memorial Scholarship
$2,000 William Andrushin, Esq. Memorial Scholarship
$2,000 BohdanWenglowskyj, Esq. Memorial Scholarship
The Memorial Scholarships will be granted based upon superlative scholastic achievement with a minimum grade of 92 for high school seniors and a 3.5 GPA for undergraduate college students, involvement in school activities, participation in community, common bond or cultural organizations, and submitted essays.
Seven (7) $1,000 Outstanding Achievement Scholarship
The Outstanding Achievement Scholarships will be granted based upon exemplary academic achievement, involvement in school activities, participation in community, common bond or cultural organizations, and submitted essays.
Five (5) $500 Merit Awards
The Merit Awards will be granted based upon notable overall academic, artistic or vocational achievement, involvement in school activities, participation in community, common bond or cultural organizations, and submitted essays.
Eligibility and Criteria
1. Scholarship applicant must be a Ukrainian FCU member in good standing and have a primary
share savings account in his/her own name for at least one (1) year .
Primary membership account must have been opened on or before March 1, 2011.
2. Applicant must be either a high school senior accepted to or currently an undergraduate, fulltime
student attending an accredited college, university or degree-granting vocational education
institution.
3. Applications will be evaluated on the basis of: two (2) essays, scholastic achievements, academic standing, and commitment to academic program, community, school or sports involvement, volunteer activities.
4. Applicant should be active in one or more UFCU common bond communities.
5. Academic, community and employer references and other specific criteria will be considered.
6. Scholarships and Merit Awards will become effective upon confirmation of attendance and
notification of acceptance by the accredited college or university.
7. Scholarship and Merit Award funds will be payable to both the educational institution and recipient and must be used during the traditional academic year for academic purposes. Undergraduate students completing studies and graduating in spring of 2012 are not eligible. Previous Ukrainian FCU scholarship or Merit award recipients are not eligible.
Application Requirements
Applicant must fully complete and sign the 2012 Scholarship Application Form.
The following required documents must be included with the fully completed application:
1. Official (signed & stamped) academic grade transcripts from attending high school, college, university or vocational education institution. Transcripts must indicate cumulative grade point average.
2. Two (2) letters of recommendation, at least one from a current teacher, counselor or school administrator. Immediate family member not acceptable.
3. Two (2) recent color photos
4. Two (2) required, original essays (600 – 800 words each) on the following topics.
a. Interesting aspects of my family history and heritage.
b. What services, benefits or factors are important to you today and in the future to keep you an active and life-long member of Ukrainian FCU?
Essay guidelines and criteria
a. Essays will be reviewed for originality, intellectual content, presentation and style.
b. Essays must be word-processed in a 12-point font, double-spaced.
c. Pages must be numbered with the last page signed by applicant.
d. Essays must include a bibliography.
e. Plagiarism will automatically disqualify application for both Scholarship and Merit Awards.
Deadline
Completed application form, two (2) essays and all required supporting documents and
information must be addressed to "Ukrainian FCU Scholarship Selection Committee", postmarked and received by the UFCU Scholarship Selection Committee at Ukrainian Federal Credit Union Main Office, 824 Ridge Rd. E, Rochester, NY 14621 on or before March 1, 2012. No late or partial applications will be accepted.
Selection of Scholarship and Merit Award recipients:
a. All applications are reviewed and evaluated by the Scholarship Selection Committee appointed by the UFCU Board of Directors.
b. Selection will be based on student's overall academic progress, extracurricular activities, participation in community activities and required written essays.
c. Applicants not receiving a Scholarship or Merit Award and those with incomplete or lateapplications are encouraged to update their applications and apply in the followingacademic year.
Notification
Applicants with incomplete and late applications will be notified March 15, 2012.
Qualifying applicants will be notified of the Scholarship Selection Committee decision by April 1, 2012.
Scholarship and Merit Award recipient schools will be notified by April 15, 2012.
Student Application Checklist
Applicant is responsible for seeing that all necessary documents are submitted in one package by
the deadline (March 1, 2012). Official (signed & stamped) transcripts and written reference letters should be submitted individually in a sealed envelope together with the application package. Name, address and phone number required for each reference.
Application package must contain ALL of the following:
1. Fully completed and signed application form
2. Official (signed & stamped) Academic grade transcripts
3.Academic Letter of Recommendation (teacher, counselor or academic administrator)
4.Non-academic Letter of Recommendation (not a relative or student)
5. Two (2) recent color photos (passport size preferred)
6. Two (2) essays (600-800 words each.
a. An interesting aspect of my family history and heritage.
b. What services, benefits or factors are important to you today and in the future to keep you an active and life-long Ukrainian FCU member.
